We would like to thank you for continuing to follow the public health and safety measures that have kept St. Joseph Catholic School safe for students and staff this school year.
To ensure a safe return to school starting on January 17th, 2022, the Ministry of Education has implemented a mandatory verification of your child’s daily self-assessment until further notice.
All Parents/Guardians are required to complete a daily self-assessment for their child(ren) and sign off daily on the verification form below and return it with their child(ren) every morning.

You must stay home if you have COVID-19 symptoms or are waiting for test results after experiencing symptoms. and complete the Form for your family every morning starting January 17th.
If your child arrives at school and verification has not been completed through the Forms link or we have not received a confirmation e-mail through the self-assessment screening tool, you will receive a telephone call and be asked to verbally confirm that the self-assessment was completed and that your child is able to attend school based on the results.
